How It Works

Step 1: Apply

Eligible low-income seniors with a physician’s order can apply by calling (616)-459-3111.

Step 2: Enjoy

Seniors and caregivers can pick up liquid nutritional supplements from the MOWWM office or have them delivered within Kent or Allegan Counties for a minimal fee.

About the Program

To sign up for liquid nutritional supplements, call our office (616) 459-3111. Please leave a message requesting a call back. We’ll return calls in the order they’re received.

How do I qualify for liquid nutritional supplements?

To qualify for liquid nutritional supplements, seniors must meet the following requirements:

They must be 60 or older.
They must live in Kent or Allegan County if they would like their liquid nutritional supplements delivered.

Seniors must also:

Have a physician’s order specifying that a liquid nutritional supplement diet is necessary and which type of supplement is needed
Have a monthly household income at or below the following thresholds:
- Household of 1: $2,510 per month
- Household of 2: $3,406 per month

How much do liquid nutritional supplements cost?

Liquid nutritional supplements are offered at reduced prices compared to retail stores. Please call us at (616) 459-3111 to learn what the current pricing is for the products we offer.

What forms of payment do you accept for liquid nutritional supplements?

We accept cash, checks, credit cards, and Bridge Cards.

Can I get my liquid nutritional supplements delivered to my home?

Yes, if you live in Kent or Allegan Counties! We charge a nominal fee for delivery, if you are not also receiving Home Delivered Meals. If you’re interested in having your liquid nutritional supplements hand delivered to your home, call our office at (616) 459-3111.

Although Meals on Wheels Western Michigan is happy to offer these liquid nutritional supplements at reduced rates, we don’t manufacture them. Your doctor or a Registered Dietitian is your best source for information about how to manage diabetes.
